Position Summary
The Community Programs & Client Services Manager is a part-time position responsible for coordinating client support services, resource navigation, and community-based programs that promote stability, health, and well-being for individuals and households served by LEAF.
This role supports clients through strengths-based engagement, basic needs coordination, and case coordination across LEAF programs, while maintaining collaborative relationships with Boulder County public agencies and community partners like the OUR Center. The position operates in alignment with Colorado nonprofit best practices, ethical standards, and confidentiality requirements.
Essential Duties and Responsibilities
Client Support & Case Management
- Provide client-centered support and care coordination to individuals and households accessing LEAF services
- Conduct client intake, needs assessment, referral coordination, and follow-up in accordance with LEAF policies
- Coordinate services across LEAF programs to promote continuity of care and reduce barriers to access
- Maintain timely, accurate, and confidential client records in compliance with organizational, ethical, and legal standards
Basic Needs & Resource Navigation
- Coordinate LEAF’s Basic Needs & Resource Matching program, including referral pathways to public and community-based resources
- Support clients in navigating Boulder County and regional systems (e.g., public health, human services, housing, food access, behavioral health, and related supports)
- Monitor service utilization and outcomes to inform internal reporting and continuous quality improvement
Community Programs & Wellness Coordination
- Support planning, coordination, and implementation of LEAF programs that promote health, wellness, and community connection
- Collaborate with staff, volunteers, and contractors to ensure programs are accessible, inclusive, and responsive to community needs
- Assist with program logistics, scheduling, and communication
Community & Public Sector Partnerships
- Develop and maintain collaborative relationships with Boulder County and regional public-sector agencies and nonprofit partners including the OUR Center
- Serve as a liaison to improve coordination of services and referral pathways for LEAF clients
- Maintain working knowledge of available public health and human-services resources affecting Lyons-area residents
Compliance, Ethics & Organizational Support
- Uphold LEAF’s mission, values, and commitment to equity, dignity, and ethical service delivery
- Maintain strict confidentiality and professional boundaries in accordance with Colorado nonprofit and human-services standards
- Participate in supervision, team meetings, training, and organizational planning as required
- Perform other duties as assigned to support organizational operations and community needs
Qualifications
Required
- Bachelor’s degree in a related field OR equivalent combination of education and experience
- Experience working in human services, public health, social services, or a related field
- Familiarity with Boulder County or Colorado-based public health and human-services systems
- Demonstrated ability to work effectively with vulnerable populations using a trauma-informed, strengths-based approach
- Commitment to ethical practice, confidentiality, and professional integrity
- Strong communication, organizational, and documentation skills
- Ability to work independently and collaboratively in a small nonprofit environment
Preferred
- Experience with case coordination, care coordination, or resource navigation
- Knowledge of rural or small-community service delivery
- Experience working in nonprofit or community-based organizations
- Bilingual (Spanish/English) skills preferred but not required
Schedule & Compensation
- Part-time, non-exempt (approximately 20-25 hours/week)
- Hourly rate: $27–$30/hour, commensurate with experience
- Flexible schedule; occasional evenings or community meetings may be required
Work Environment
- Combination of office-based, community-based, and partner-facing work
